Condition / Assessment
Condition Score
A structured way to examine how well organizational knowledge holds up as it moves between people, systems and AI.
Score looks at what is known, where it came from, who is responsible for it and what happens when it is used.

What We Examine
Four conditions of useful knowledge.
Condition Score examines the underlying conditions that determine whether information can be understood, trusted and used without losing what matters.
01
Structure
Can it be understood?
Is information organized well enough to be found, related and interpreted in context?
02
Governance
Is responsibility clear?
Can you tell who may create, change, approve, use and retire important information?
03
Provenance
Can it be trusted?
Can important claims be traced to their source, authority, history and applicable context?
04
Evaluation
Does it hold up in use?
Does the information produce the intended result when people and systems actually rely on it?
What It Produces
A record of what holds up, and what doesn't.
Score turns an assessment into something inspectable: findings, evidence, unresolved questions and the relationships between them.
01 / Condition
Condition Profile
A view of how the assessed knowledge performs across structure, governance, provenance and evaluation.
02 / Findings
Finding Register
What held up, what failed and what remains uncertain.
03 / Sources
Source Map
The material supporting important findings and where authority originates.
04 / Gaps
Knowledge Gaps
Places where the organization cannot currently establish a reliable answer.
05 / Decisions
Decision Record
Judgments made during assessment, including why they were made and where they apply.

Reading The Assessment
How to read a Condition Score.
The assessment is not a verdict. It's a map of what can be established.
Established
Supported by sufficient evidence and clear applicability.
Contested
Relevant sources, claims or decisions disagree.
Unclear
Available evidence does not support a confident judgment.
Untested
The information exists, but we do not yet know how it performs in use.
Common questions:
What does Condition Score assess?
The information an organization depends on and the systems that use it. The scope can range from a defined body of knowledge to a particular product, workflow or AI system.
Is this a content audit?
Not exactly. Content may be part of the evidence, but Score is interested in what sits underneath it: sources, claims, ownership, decisions, change and use.
Does every assessment produce a numerical score?
No. A number is only useful when the underlying measurement justifies one. Condition prioritizes findings and evidence over false precision.
What happens when the answer isn't clear?
We say so. Uncertainty is recorded rather than converted into certainty for the sake of completing the assessment.
Can Condition Score change?
It should. The method develops as Condition's research produces better evidence about what matters and how it can be evaluated.
Find out what your systems are relying on.
Condition Score examines the knowledge underneath a system before assuming the system itself is the problem.
